| Jaguar Land Rover Limited (H570/571/572) | Motor Vehicle | I-PACE 2019 – 2021 Model Year vehicles | Vehicles have experienced thermal overload which may show as smoke or fire, that may occur in the high voltage traction battery pack. The investigation is ongoing. Modules that were identified by the remedy software as having characteristics of a folded anode tab, which may contribute to a risk of thermal overload, are still being inspected by the supplier. Cell inspection and testing indicate that anode tab folding can contribute to short circuits, but it is likely that other conditions also need to be present to excite the thermal overload condition. At this time, such potential secondary causes have not been determined. The supplier investigation continues to seek other conditions through component inspection and testing. Since the installation of protective software, field evidence has shown that there remains a risk of thermal overload in vehicles with battery packs manufactured up to the end of 2021 model year. | A vehicle thermal overload condition such as fire or smoke can result in increased risk of occupant injury and/or injury to persons outside the vehicle, as well as property damage. | 4/1/2026 | 4/2/2026 | 4/2/2026 | 2 |

The investigation identified no device malfunctions, manufacturing nonconformities, or performance deficiencies. System operating parameters were within expected ranges, and the reported events are consistent with known procedural risks associated with cryoablation therapy, including thermal injury mechanisms and contributing procedural factors such as repeated applications and thermal stacking in the left pulmonary veins. The observed atrio-esophageal fistula (AEF) occurrence rate (0.0103%) remains within anticipated levels defined in the risk management documentation and within ranges reported in clinical literature. No compliance deficiencies have been identified. While AEF remains a rare complication, it represents the most serious potential patient harm associated with the procedure (Severity level 5). The occurrence of recent events prompted continued evaluation of patient risk and the occurrence rate has remained low but variable in both the overall population and the sub-population since August 2024. The three (3) recent events result in an overall rate that is similar to the rate prior to the October 2024 FSCA. Although the device continues to perform as expected within current risk documentation, alternative technologies to cryoablation such as Pulsed Field Ablation (PFA) are available and carry a lesser risk of thermal injury and its potential consequences (e.g. AEF). As a result, patients may benefit from alternative treatment strategies. | 4/9/2026 | 4/9/2026 | 4/10/2026 | 2 |
